A wooden-based plaque with a plastic-topped certificate on the front. The certificate is very simple, it is printed in black ink with some golden letters and words. At the bottom, it is signed by the president, the secretary, the Chairman, and the Board of Trustees. At the bottom of the certificate, there is the emblem of the Illinois State Medical Society: a caduceus in the outlined state of Illinois.

Donated by the Estate of Dr. John N. Nicholson. Plaque was awarded to Dr. John N. Nicholson for being a part of the 50-year club at the Illinois State Medical Society. Dr. Nicholson received his medical degree in 1932 and was initiated into the Fifty-Year Club on April 17, 1982.
