This is an ivory colored program with brown printing. There is a brown tassel attached to the program. Inside, there is a clear piece of paper on which is a picture and a biography of Hippocrates. On the back inside cover lists the people who are associated with and involved in the unveiling of this statue.

The unveiling of the Hippocrates statue took place at the University of Illinois at Chicago Hospital in 1980.
