Object Description
A certificate of death printed on white paper in black ink. Some things are typed in a type-writer font and other things are written (the copy seems to be photocopied). At the left of the paper in the middle there is an indentation in the paper where Cook County stamped it with a colorless, circular stamp.
Origin
The Death Certificate of Chrisanthe Calas. Chrisanthe died at American Health Care Center in Arlington Heights, Illinois of Accute Cardiopulmunary arrest due to her heart disease. She died at 3 p.m. on May, 19 1985. Chrisanthe was born in Crete, Greece to Theano and Nicholas Mazokopakis on February 21, 1903. She came to the United States in 1926 and married an honorably discharged American soldier, Andrew Calas (who was already deceased at the time of her passing). They had a son named Andrew. Before her death, Chrisanthe lived at 512 Kinkaid, Des Plaines, Illinois. She was buried in Elmwood Cemetery, River Grove, Illinois on May 22, 1985. The certificate of death was signed on May 21, 1985 by a Cook County Deparment of Public Health Official Cheif Registratar.
