The document has an elaborate black-and-white border that encases the central text. The border is composed of a leafy design seen in each corner of the document, an omental design in the central edges encased in a decorative webbing with the word "Department of Labor" at the central bottom. The center of the document uses cursive font and printed text. In the center of the document is the word "Original." The certificate has multiple staples, approximately four, on the left side, which attach the certificate to five additional documents.
