Large brown leather bound record book. There are creases/wrinkles in the material that are black in color. On the spine of the case there is a light green design: the very top is a square with flowers pointing inward in the corners and a floralish like pattern in the very center. There are two horizontal lines under that and about an inch gap before another two. Then we have the same floral patter as before with another two lines under another inch gap and a repeat of the same pattern. There is one streak of white paint splashed on the cover, close to nine inches long and a few small splatters here and there on both sides.
The inside cover is lined with cardboard. Stamped on the inside is the Index. Index is printed in the top center with a spread sheet withe 'Page' Record Number' Title' and Artist'. There are twelve page slots, two record number, title, and artist per slot. There is nothing written in any spot. Underneath the spread sheet is "Made in USA. Manufactured By. Chicago Album & Specialty Co." printed in three lines.
There are twelve cardboard pages that hold the records. The top of the page is curved slightly inward to make it easier to grab the record and there is a circle cut out of the center to allow one to read what the record was. This circle was cut out on both sides of the page. Although there are twelve pages, only eight of them are currently holding records.
On the very first page, in pencil is written "96-865"
